HOW TO CHOOSE CARESOURCE
Medicaid health care coverage is available for eligible Ohioans with low income, pregnant women, infants and children, older adults and individuals with disabilities. 秋葵app Medicaid is available across the state of Ohio.

When you apply for Ohio Medicaid, you can choose 秋葵app as your managed care plan. Whether you apply online, by mail, over the phone or at your local Department of Job and Family Services (JFS), tell them you want to choose 秋葵app. |

If you were assigned to another managed care plan and want to switch to 秋葵app, call the Ohio Medicaid hotline, complete a change request within 90 days and tell them you want to switch to 秋葵app. If you have been on your plan more than 90 days, you can switch to 秋葵app during your next open enrollment period. |

Ohio Medicaid’s managed care plans are designed to provide personalized care to meet your healthcare needs. You can explore the managed care plans available and enroll by visiting or by contacting the Ohio Medicaid Consumer Hotline at (800) 324-8680. Representatives are available 7 a.m. to 8 p.m. Eastern time (ET) Monday through Friday and 8 a.m. to 5 p.m. ET Saturday. They can provide interpreters and answer member questions about open enrollment.
